State Police in Warsaw are investigating a one vehicle accident that occurred on West Middlebury road (Bank road) on Wednesday evening.
State Police say David L Coulter, 76, of Wyoming, died as a result of the accident. Coulter was transported by Mercy Flight from the scene and died later on Wednesday night.
State Police are investigating the cause of the accident.
